Medical information

Share what helps. Protect what does not need to be shared.

ResQCare's current request form collects a short patient reference, emergency category, resource needs and an optional note.

Your information

Keep an emergency request focused.

Use initials or a short reference instead of a full name. Select the category and resources the hospital should prepare for. Include only a brief note if it improves coordination.

Do not enter identity numbers, payment details or a full medical history in the request note. If a clinician needs more detail, follow their instructions directly.

No medical profile is stored here. The existing system does not provide a patient medical-record or profile feature. This page offers guidance without creating a new database or storing health details in your browser.

Personal details

Use a short patient reference in a request. The current platform does not store a full verified patient profile.

Blood group

Keep your verified blood group information available to clinicians. It is not stored in the current ResQCare request model.

Medical conditions

Tell emergency clinicians directly about relevant conditions. Do not put a full medical history in an unverified request note.

Allergies

If an allergy affects immediate care, communicate it directly to the care team as soon as possible.

Current medications

Keep an accurate medication list with you and share it with clinicians through their approved process.

Emergency contact

Keep a trusted contact available on your own device. The current ResQCare request model has no protected contact record.

Important notes

Use the optional request note only for concise coordination details needed before arrival.
